This session will provide an opportunity for valuable networking with the Practice Facilitators currently supporting practice transformation in stage one implementation of the Health Care Homes model in Australian primary care. PHN Practice Facilitators and Health Care Homes trainers will share their experiences and resources on how they have supported practices to build their capacity, prepare the practice team for change management, and facilitate incremental improvements in the quality of care provision for patients with chronic complex conditions.

Kristianne Jarman - BIO


Kristianne Jarman is a registered nurse and credentialed diabetes educator, and has worked in primary health care for over ten years.  In her previous role at Diabetes Queensland, she was a Project Officer for health professional education and prior to this she assisted in the establishment of three metropolitan GP clinics.  As Practice Facilitator and Quality Improvement Coach in her current role at AGPAL, Kristianne will draw on her clinical experience and extensive knowledge of primary care to provide coaching to Primary Health Network Practice Facilitators in supporting Health Care Homes. Achieving optimal health outcomes through the facilitation of quality improvement and innovation is something Kristianne is extremely passionate about.
