How to becoming a high performing general practice or Aboriginal Community Controlled Health Service


This interactive workshop will incorporate mini sessions on relevant hot topics that will support practice teams and PHN Practice Facilitators on their journey of becoming a high performing general practice or ACCHS. It will draw from the ten building blocks of the high performing primary care framework and other evidence based methods for implementing change to achieve the Quadruple Aim of health. Through these sessions, participants will gain insight, have the opportunity to ask questions and workshop ideas. This workshop will build on the AGPAL stage one Health Care Homes training or Health Care Homes Readiness training.

Melissa Simpson - Bio

Melissa is delighted to be offered the chance to talk at this conference. No stranger to primary health care, Melissa is a Registered Nurse, and worked in a busy suburban General Practice for over half of her life. During this time, she completed her Post-Graduate Diploma endorsed with Long Term Conditions.

Moving to Compass Health in 2015, she became a LEAN practitioner, more recently taking on the role of Health Care Home Programme Lead. Both of these roles are stimulating, offering as they do the opportunity to implement a new model of care essential for the future proofing of General Practice, whilst – crucially -  staying responsive to the needs of patients.

Melissa will be sharing her experiences about the Health Care Home model of care, and the transformational  journey that practice’s experiences
